Waverly Health Center's Home and Community Health program brings experienced health care professionals into your home - 24 hours a day, 365 days a year.
Our services provide quality, comprehensive in-home health services that are designed to decrease the need for institutional care and help promote patient independence.
Waverly Health Center's Home and Community Health program is Medicare/Medicaid certified, JCAHO accredited, and covered by most private insurance Companies. Services of WHC's Home and Community Health program are available within a 30 mile radius of Waverly. Waverly Health Center Home and Community Health is an elder, ill & handicapped & MR waiver provider.

Assessment of the patient's condition
Care of wounds and changing of dressing

Flu & pneumonia immunization clinics
Equipment loan closet
Blood pressure screening
Infant & child car seat safety program
Lead screening
Tobacco Prevention Program - provides community awareness & education in the prevention & cessation of tobacco use
Community Health Outreach Network - provides assessment of the health care needs of school-age children, referral of these needs to appropriate area service providers and delivery of necessary health care services to this population at their school site. All services are coordinated and delivered through a unified community health care network.
