If you were just diagnosed with diabetes, have uncontrolled blood sugar levels, or want to learn more about your diabetes, we can help.

Services

Diabetes Self-Management Education and Support Program

This program is nationally recognized and accredited by the American Diabetes Association. You will learn about:

  • Being active
  • Healthy coping
  • Improving eating habits
  • Monitoring blood glucose
  • Problem-solving
  • Reducing risks
  • Taking medicine

Our team will partner with you, your family and your health care provider so you can be healthy with diabetes.

We offer individual counseling with flexible scheduling. Please call us for more information.

Prediabetes

Prediabetes means you have blood glucose (sugar) levels that are higher than normal, but not high enough to be called diabetes. Recent research has shown that some long-term damage to the body, mainly the heart and circulatory system, may be taking place during prediabetes. Learn what you can do to prevent or delay the development of type 2 diabetes.

Become a Patient

Your health care provider can order these services for you. We accept Medicare, Medicaid and most medical insurances. Medicare covers up to 10 hours of diabetes education in the first 12 months after a referral. Medicare covers up to two hours of “refresher” education each year after that. Please verify with your insurance company that your test or treatment is a covered benefit.
